Our 24/7 call center, powered by digitalDONOR®, ensures that every referral is properly vetted, documented and assigned to a Care Coordinator, as appropriate. digitalDONOR guides our medical call center staff through the examination and screening of organ and tissue referrals based on your organization’s criteria. digitalDONOR’s workflows and decision support capabilities maximize your success rate. Our automated notification processes ensure that your scheduled staff is alerted, receives the pertinent information and is on the case within seconds after determining eligibility. Additionally, referring facilities are provided with documentation of the call and its results, as well as provided with the "who, what, when and where’" related to the next steps in the process. Care Coordinator Assistance
The critical nature of the work performed by Transplant Care Coordinators cannot be underestimated. These individuals must have top-shelf credentials when it comes to psychosocial, clinical and administrative expertise to successfully manage the continuum of activities that ultimately result in successful organ transplantations. Obtaining consent from next of kin, successfully recovering organs and tissue, ensuring accurate matching, and organ and tissue acceptance are all mission-critical steps in a successful transplantation. We understand that it is the rare individual who can juggle these imperatives, while the clock is ticking and lives are at stake.
Our Care Coordinator Services assume the administrative responsibilities associated with Care Coordination by assigning a Care Center team member trained specifically to support your staff. Using digitalDONOR as their "checklist" and donor management system, your Care Coordinators simply contact Kinective via cell phone and dictate their activities to our staff, who enter the data into the patient’s record. This dialogue includes reminders of incomplete documentation so that the onsite Care Coordinator may ensure that the donor documentation is accurate and complete. This facilitates successful matching and ultimately transplantation. Our Care Coordinator Service eliminates paper from the process; affords digitalDONOR’s decision support tools, configured to your processes and requirements; and ensures that all necessary documentation is acquired and uploaded to UNOS for matching.
